Bespoke Naturopathic Medicine is a health information custodian under Ontario's Personal Health Information Protection Act (PHIPA). Dr. Kimberly Boileau, ND, is responsible for how your information is handled.
Your name and contact details, date of birth, health history, symptoms, medications and supplements, lab results, visit notes, and payment and insurance details — collected from you, from tests you complete, and, with your permission, from other providers.
To book and provide your care, communicate with you about your care, bill you and your insurer, and meet our legal and professional obligations. We do not sell your information.
Your care team at Bespoke (naturopathic doctors, nurse practitioners and clinic staff involved in your care), and other healthcare providers you ask us to share with. We may be required to disclose information where the law requires it, such as to our regulatory college or a court.
Your records are kept in Practice Better, a secure, encrypted electronic health record. Access is limited to authorized team members, and we retain records for the period required by our regulatory college.
You may ask to see or correct your record, limit who at Bespoke can access it, or withdraw your consent to collection or sharing (which may limit the care we can provide). You may also contact the Information and Privacy Commissioner of Ontario if you have a concern we cannot resolve.
If you opt in, we'll send you health insights, program updates and occasional offers from Bespoke Naturopathic Medicine. Your email consent is separate from your care — you can decline it and still book with us. Every email includes an unsubscribe link, and you can withdraw consent at any time by clicking it or contacting the clinic. Appointment reminders and care-related messages are not marketing and will continue regardless.
Contact Dr. Kimberly Boileau through the clinic at 8 Court Street South, Thunder Bay, or via our contact page.
